A new initiative in Alabama allows individuals and businesses to support rural hospitals while reaping state tax benefits. This dollar-for-dollar credit benefits crucial healthcare resources in rural communities across the state while providing a meaningful financial incentive for taxpayers.
Alabama’s Rural Hospital Investment Program begins January 1, 2026, and continues through the end of 2028, with many hoping the program will be extended.Â
Donor Eligibility and Credit Limits
The Rural Hospital Investment Credit can offset many types of state tax liabilities, including state income tax, financial institution excise tax, insurance premiums tax and utility taxes paid. The amount of the credit is equal to 100% of the amount of qualified donations, with annual limits that vary based on the donor’s filing status and entity:
- Up to $15,000 for individuals filing single, head of household or married filing separately.
- Up to $30,000 for married individuals filing jointly.
- Up to $450,000 for passthrough entities.
- Up to $500,000 for corporations based on 100% of the amount of the donation or 75% of income tax liability, whichever is less.
- Up to $500,000 for financial institutions based on 100% of the amount of the donation or 75 % of the institution’s Alabama excise tax liability, whichever is less.
- Up to $500,000 for insurance companies based on 100% of the amount of the donation or 75% of the company’s insurance premium tax liability, whichever is less.
- For utility taxpayers with a utility tax direct pay permit, the credit is limited to the lesser of 100% of the amount of the donation or the total utility taxes paid.Â
Limited Availability
A limited number of credits are available statewide, with per-hospital caps in place as well. There’s also a cap on the dollar amount of credits: Up to $750,000 per eligible rural hospital and no more than $20 million in total credits for facilities across Alabama in 2026. Those figures go to $25 million in 2027 and $30 million in subsequent years for the statewide aggregate cap on investment. Caps on tax credits that go to individual facilities rise to $1 million in 2027 and $1.25 million in 2028.
Healthcare facilities that want to participate in the program must seek approval from the Rural Hospital Investment Program Board within the Office of the State Treasurer. The board classifies approved hospitals into five tiers that reflect varying degrees of financial need. The state has created a Rural Hospital Investment Program Operations Manual that offers detailed guidance about participation and plans for how funds will be distributed.
Act Now! The Process for Donors
There’s a relatively simple set of guidelines to help potential donors navigate the program and claim the associated tax benefits. Step one is reserving your credit online through the Alabama Department of Revenue. Timely action is important for businesses and individuals interested in participating, because these limited credits are awarded on a first come-first serve basis.Â
Credit reservations are currently open and can be made on your My Alabama Taxes Portal.Â
Donors must make monetary donations directly to the eligible rural hospital or an approved third-party organization within 30 days of reserving a credit. (Only monetary donations count toward the program; no services, goods, debt forgiveness, or other forms of gifts are eligible.) It’s fine to make donations that are larger than the reserved amount, but only the reserved amount will count toward a tax credit.Â
The receiving organization must certify receipt of the donation within 30 days. This is an essential responsibility, because uncertified donations are redirected to the state’s annual fund with no tax credit awarded.Â
Properly reserved, received and certified donations apply to the tax year in which the donor reserves the credit, with unused portions of the credit carrying forward for up to three years. Alabama Rural Hospital Investment Tax Credits are nontransferable.
